Application for OMS Tour

To apply for this tour, simply fill in all required fields in the form below. All fields with an * are required. Please note that the tour works on a first-come-first-serve basis. After you submitted your application, we will notify you by e-mail within 24 hours whether you have been admitted for the tour.

Instructions

1. First name: Please enter your first name
2. Last name: Please enter your last name / family name
3. E-Mail: Please enter your e-mail address
4. Phone: Please enter your phone number (this is not mandatory)
5. Course I: Please enter the first course, in which you receive credit; example: "MGMT 6100 - Operations Management"; enter "none" if you do not receive credit.
6. Professor I: Please enter the name of the professor of the course in which you receive credit; example: "Dr. Radovilsky" enter "none" if you do not receive credit.
7. Course II: Please enter the second in which you receive credit, if applicable; otherwise, leave the field blank.
8. Professor II: Please enter the name of the professor of your second course, for which you receive credit, if applicable; otherwise, leave the field blank.
9. Only for OMS members: Please enter your OMS membership number (voluntarily).

You will receive an error message, if you don't fill out required fields. In this case, please go back, and re-enter your data.

If you fail to correctly enter your course number and/or the name of your professor, you will receive no extra credit.
